What omega-3 supplements actually are

Omega-3 is a family of fatty acids. The long-chain forms EPA and DHA are the ones most often discussed in relation to omega-3 supplements; they are found in marine oils such as fish oil, krill oil, and algae oil. ALA is a shorter-chain, plant-derived omega-3 from sources such as flaxseed.

The single biggest source of confusion on omega-3 labels is the difference between the total fish oil weight and the EPA + DHA amount. A “1,000 mg fish oil” softgel may contain only a few hundred milligrams of EPA and DHA combined — and EPA + DHA is the number that lets you compare two products fairly.

This hub does not recommend an omega-3 source or dose for heart, brain, eye, or any other outcome. It explains what the labels mean and how to compare products on declared facts.

These names describe the source and the chemical form of the oil. The number that matters for comparison is the EPA + DHA amount per serving — not the total oil weight.

Fish oil
The most common omega-3 source, providing EPA and DHA. Concentration varies widely: compare the EPA + DHA per serving, not the total fish oil weight.
Krill oil
Sourced from krill; EPA and DHA are carried in phospholipids rather than triglycerides. A different presentation — compare the declared EPA + DHA amount as always.
Algae oil
A marine microalgae source of EPA and DHA that is vegan. Useful for people who avoid fish; the comparison is still on the declared EPA + DHA per serving.
Cod liver oil
An oil that provides EPA and DHA alongside vitamins A and D. The presence of fat-soluble vitamins means reading the full label matters before comparing.
Triglyceride vs ethyl ester
The chemical form the oil is carried in. Both can deliver EPA and DHA; the form is a processing detail that sits behind the number that actually matters — the declared EPA + DHA amount.

Omega-3 is a family of polyunsaturated fatty acids. The long-chain forms EPA and DHA are found in marine oils and are incorporated into cell membranes throughout the body. ALA is a shorter-chain plant-derived omega-3 (from flaxseed, chia, walnuts) that the body converts to EPA and DHA only in small amounts.

Omega-3 fatty acids are studied in relation to normal heart, brain, and eye function, and to the body’s inflammatory responses. Marine-sourced EPA and DHA are the forms most commonly discussed in relation to supplements.

Omega-3 fatty acids are among the most studied nutrients, but “omega-3” is not one homogeneous intervention. Results can differ according to EPA vs DHA, formulation, dose, population, background therapy, and the outcome measured. The summary below orients you using an explicit evidence hierarchy; it is not a treatment or dosing recommendation, and large trials have produced mixed results across outcomes.

What the research landscape looks like

  • Evidence hierarchy — why the layer matters

    When reading omega-3 research, distinguish five layers: (1) mechanistic/biological findings (how EPA and DHA act in cells), (2) blood fatty-acid or inflammatory biomarkers (measurable changes in the body), (3) surrogate outcomes such as triglycerides (a risk marker, not a disease event), (4) clinical cardiovascular outcomes such as heart attack, stroke, or death, and (5) formulation- and population-specific evidence. A change in a biomarker or surrogate is not the same as a proven clinical outcome. This hub does not convert biomarker findings into disease-prevention claims.

  • Dietary reference values

    EFSA sets an adequate intake of 250–500 mg of EPA+DHA per day for adults. The US has an adequate intake for the plant-derived omega-3 ALA (1.6 g/day for men, 1.1 g/day for women) but no separate RDA for EPA+DHA.

  • Marine vs plant sources

    Fish, krill, and algae provide EPA and DHA directly. Plant ALA must be converted, and conversion to EPA and especially DHA is limited. This is why vegan diets often rely on algae oil for direct EPA and DHA.

  • Studied outcomes

    Omega-3s have been studied for cardiovascular, cognitive, eye, and inflammatory outcomes. Results vary by population, dose, formulation, and baseline status; no outcome is established here as a supplement effect.

  • 2026 moderate-dose EPA+DHA cardiovascular meta-analysis (PMID 42144851)

    A 2026 pooled analysis of 25 RCTs and 25,578 participants found no statistically significant reduction in major adverse cardiovascular events (MACE) and no statistically significant reduction in atrial fibrillation or post-operative atrial fibrillation (AF/POAF) with combined EPA+DHA in the included secondary-prevention and perioperative populations. This result concerns combined EPA+DHA in those specific populations and should NOT be generalized to every omega-3 product, dose, population, or clinical indication. It is one reason broad claims such as “omega-3 prevents heart disease” are too simplistic.

  • 2026 formulation-specific high-dose EPA evidence (PMID 42070013)

    A 2026 analysis of formulation-specific cardiovascular outcomes with high-dose eicosapentaenoic acid (EPA) found that high-dose EPA-dominant formulations have a different evidence base from mixed EPA+DHA supplements. This hub does NOT say that prescription or high-dose EPA results prove that ordinary fish-oil supplements prevent cardiovascular events.     A 2026 systematic review and meta-analysis of 96 clinical trials indicates that the EPA:DHA ratio and total dose can influence blood fatty-acid profiles and inflammatory biomarkers. These are biomarker and biological-response findings. This hub does NOT convert them into claims that a particular EPA:DHA ratio is clinically superior for preventing disease. A biomarker response is not a proven clinical outcome.

Reference values cover total intake from food and supplements combined. Some bodies advise that supplemental EPA+DHA above 3–4 g/day be used only under medical supervision. These are not personal dosing instructions.

Safety & side effects

Omega-3 supplements are generally well tolerated at typical amounts. The most commonly reported side effects are mild gastrointestinal — fishy aftertaste, reflux, and loose stools — which splitting doses and taking with food can reduce.

High-dose EPA+DHA (well above typical supplement amounts) can affect blood clotting and has been studied for interactions with anticoagulant medications. Authoritative bodies generally advise that high-dose omega-3 supplementation be supervised by a healthcare professional.

A dose- and baseline-risk-dependent atrial-fibrillation signal has also been reported at higher omega-3 doses (see the safety considerations below). This is not a reason to avoid typical supplement amounts, but it is a reason to discuss higher-dose use with a clinician if you have existing atrial fibrillation or elevated cardiovascular risk.

  • Blood thinners and clotting

    Very high omega-3 intakes can have a mild effect on blood clotting. People on anticoagulant or antiplatelet medication should consult their doctor before high-dose omega-3 supplementation.

  • Atrial fibrillation — a dose- and baseline-risk-dependent signal

    A 2026 updated meta-analysis (PMID 42517224; 35 RCTs, 114,592 participants) reported a statistically significant increase in incident atrial fibrillation in the high-cardiovascular-risk / >1.5 g/day EPA+DHA stratum, with a pooled odds ratio of 1.43 (95% CI 1.14–1.79) and an absolute risk difference of approximately 0.8%. The other three dose/risk strata did not show statistically significant increases. This is a dose- and baseline-risk-dependent signal at higher doses — it should NOT be simplified to “omega-3 causes atrial fibrillation.” People with existing atrial fibrillation, a history of AF, or elevated cardiovascular risk should discuss higher-dose omega-3 use with their clinician before starting.

  • Marine contaminants

    Marine oils can carry heavy metals and oxidised lipids. Independent testing (e.g. IFOS) and certifications (e.g. Friend of the Sea) are relevant quality signals; HerbParlor records what is reported.

  • Fish allergy

    People with fish or shellfish allergy should approach fish- and krill-derived products with care; algae oil is a marine but non-fish alternative worth discussing with a clinician.

Anticoagulants and antiplatelets
High-dose EPA+DHA may add to the effect of blood-thinning medications. Medical supervision is advised when combining them at high doses.
Blood pressure medications
Omega-3s have been studied for effects on blood pressure. Anyone on antihypertensive medication should discuss omega-3 supplementation with their clinician.
Weight-loss drugs (orlistat)
Orlistat reduces fat absorption and may lower absorption of omega-3 fatty acids taken at the same time; dose separation is commonly suggested.
Other marine-oil supplements

No single omega-3 product is “best” for everyone. These neutral criteria help you narrow the field to the products worth comparing side by side. Some products lean toward EPA, others toward DHA, and many balance the two. The EPA:DHA split is a label fact, not a quality verdict — decide which split is relevant to your goal, then compare products that disclose it.
Formulation identity — EPA-dominant vs mixed EPA+DHA
High-dose EPA-dominant formulations have a different evidence base from ordinary mixed EPA+DHA fish-oil supplements. Where a label discloses it, note whether the product is EPA-dominant or balanced EPA+DHA, and the EPA:DHA ratio. Total EPA+DHA per serving
