Fibrinolytic activity is studied because fibrin is part of the body’s normal clotting and clot-breakdown balance. It is important to keep four levels of evidence distinct: (1) mechanism — what nattokinase does in a laboratory; (2) biomarker or surrogate outcomes — changes in measured blood values such as blood pressure, fibrinolytic markers, or lipids; (3) intermediate physiological outcomes; and (4) clinical outcomes — actual cardiovascular events such as stroke, heart attack, or venous blood clots. A change at one level does not automatically establish an effect at a higher level. A blood-pressure reading that moves in a study is not, by itself, proof that nattokinase prevents cardiovascular disease.
Because of this gap, cardiovascular marketing claims for nattokinase can exceed the clinical evidence. Terms such as “blood thinner,” “blood pressure support,” “circulation support,” “clot-busting,” or “heart health” are not independently defined or regulated treatment claims, and this hub does not present them as established clinical outcomes. What the research landscape looks like
Mechanism — fibrinolytic activity in the laboratory
Nattokinase is described in scientific literature as a fibrinolytic enzyme, and laboratory and mechanistic studies have examined its activity related to fibrin breakdown. These findings describe a biochemical property under controlled conditions; they are the starting point for research, not established clinical outcomes, and they do not show that a nattokinase supplement treats, prevents, or cures blood clots or any condition in people.
Blood pressure measurements (human studies)
