What iron supplements actually are
Iron is an essential mineral the body cannot make, so it must come from food or supplements. It is found naturally in animal-derived foods such as meat, fish, and poultry (haem iron) and in plant foods such as legumes, lentils, tofu, spinach, and fortified cereals (non-haem iron), and is added to some fortified foods. Government dietary references describe how much iron people generally need from all sources combined — food and supplements together — and those references are a useful backdrop, not a product recommendation.
An “iron supplement” is usually one of several iron compounds — ferrous sulfate, ferrous gluconate, ferrous fumarate, ferrous bisglycinate, carbonyl iron, polysaccharide iron complex, or heme iron — presented as tablets, capsules, or liquids. Two products can both say “iron” and differ in form, in the elemental iron per serving, and in the format that delivers it. Labels normally declare the amount of elemental iron (the iron itself), not the weight of the whole compound.

These names describe the chemical form of iron and the delivery format. The number that matters for comparison is the declared elemental iron per serving — stated consistently in the same unit.
- Ferrous sulfate
- The most common and most studied iron salt, often the reference form in research. Compare the declared elemental iron per serving, not the compound weight or the form name alone.
- Ferrous gluconate & ferrous fumarate
- Further iron salts that appear on many labels. Each is a different compound with a different proportion of elemental iron — compare the declared elemental iron amount and the documentation behind the product.
- Ferrous bisglycinate / glycinate
- A chelated form of iron bound to the amino acid glycine, often marketed as “gentle” or better tolerated. The comparison is still on the declared elemental iron per serving and the testing behind the product.
- Carbonyl iron

- Polysaccharide iron complex (PIC)

Iron: health benefits & biological roles
Iron is an essential mineral found naturally in animal-derived foods — meat, fish, and poultry — as haem iron, and in plant foods such as legumes, lentils, tofu, spinach, and fortified cereals as non-haem iron, and is added to some fortified foods. It is a core component of haemoglobin (the protein in red blood cells that carries oxygen) and myoglobin (the protein that stores oxygen in muscle), and it is required for normal energy metabolism, normal cognitive function, and a normally functioning immune system.
Because the body has no regulated way to excrete excess iron, status depends on intake and absorption, and absorption is tightly controlled. Non-haem iron from plant foods is less readily absorbed than haem iron from animal foods, and absorption can be increased by vitamin C or reduced by compounds in tea, coffee, and some plant foods. Iron deficiency is one of the more common nutrient shortfalls worldwide and is most often discussed for menstruating women, pregnant women, young children, and people with restricted diets or absorption problems.

The US RDA for iron is 8 mg/day for adult men and 18 mg/day for adult women aged 19–50, with 27 mg/day in pregnancy and 9 mg/day in lactation, and a tolerable upper intake level of 45 mg/day for adults. EFSA sets population reference intakes ranging from about 9 to 16 mg/day depending on age, sex, and life stage.
Iron forms and absorption
Most research uses ferrous sulfate. Chelated forms (bisglycinate), carbonyl iron, polysaccharide iron complex, heme iron, and liposomal preparations are different compounds; studies comparing their absorption, tolerability, or effect have produced varying results, and no form is established here as superior for any outcome.

Iron has been studied for deficiency, iron-deficiency anaemia, pregnancy outcomes, fatigue, and exercise performance, among others. Reference values cover total intake from food and supplements combined. Menstruating and pregnant women have higher requirements; postmenopausal women and adult men have lower requirements. Iron supplementation has meaningful safety considerations that other common minerals do not. The body has no regulated way to excrete excess iron, so too much iron can accumulate, and acute iron overdose is a medical emergency — it is a leading cause of poisoning deaths in young children. Keep iron supplements out of the reach of children, in child-resistant packaging.
At typical supplemental amounts, the most common side effects are gastrointestinal: constipation, nausea, stomach pain, and dark-coloured stools (which are normal with iron and not a cause for concern on their own). Taking iron with food can reduce nausea but also reduces absorption; spacing iron away from tea, coffee, and calcium-rich foods improves absorption.
People with iron-storage disorders such as hereditary haemochromatosis, or who regularly receive blood transfusions, should not take iron supplements unless advised by a clinician. Because iron status is best assessed with a blood test, supplementing iron “just in case” is not advisable — excess iron carries real risks.
Acute overdose and child safety
Adult-strength iron doses can be dangerous, even fatal, to young children. Store iron supplements securely and in child-resistant packaging, and seek emergency medical attention immediately if a child takes iron supplements accidentally.
Gastrointestinal effects
Constipation, nausea, and stomach pain are common with oral iron. Some people find certain forms or lower single doses better tolerated; this is individual and not a quality verdict on a form. Dark stools are normal with iron.
Iron overload and haemochromatosis
People with hereditary haemochromatosis or other iron-storage conditions, and people who receive regular blood transfusions, should not supplement iron unless a clinician advises it. Excess iron can accumulate and damage organs over time.

- Calcium
- Calcium can reduce iron absorption. Spacing iron and calcium supplements (or calcium-rich foods) apart is a common precaution, especially for people taking both.
- Tea, coffee, and tannin-rich foods
- Tannins and other compounds in tea and coffee can reduce non-haem iron absorption. Spacing iron away from these drinks is commonly suggested.
- Vitamin C
- Vitamin C increases the absorption of non-haem iron. Some products pair the two for this reason; discuss whether that applies to you with a healthcare professional rather than self-combining at high doses.
- Levothyroxine
- Iron can reduce the absorption of levothyroxine (thyroid medication). Spacing iron and levothyroxine several hours apart is commonly advised; check with the prescribing clinician for your specific regimen.
- Certain antibiotics (tetracyclines and quinolones)
- Iron can bind to some antibiotics and reduce their absorption. Spacing iron two hours before or after these antibiotics is commonly advised; check with a pharmacist for your specific medication.
- Zinc
- High-dose iron and zinc can compete for absorption when taken together. Spacing them apart is a common precaution, especially for people taking both at higher doses.
